1/10/2023 EnchantaVaughn Approved Client was seen preparing for morning lesson by putting away his belongings and facing forward. Client displayed appropriate behaviors while listening to the announcements. Client was attentive when his teacher was instructing the class. Client was respectful to his classmates and giving them good common courtesy when it was not his turn to talk. Client was encouraged to have a good day and get his work done. Client reviewed his morning assignment with his counselor and was confident that he knows what to do. Client was given a thumbs up for having a positive attitude and being productive. Client displayed high level of participation during educational and fundamental activities throughout the day. Client displayed cooperative behaviors while listening to authority prompts and demands. The client was able to go to lunch without feeling the need to act out. Client was observed in this setting interacting well with his peers. Client is heavily influenced by his peers, when they are working he tend to be working and when they are off task he follows right behind them. Client chose to follow behind the good behaviors today. Client transitions to speak with mental health professional after lunch. Client symptoms of ADHD have been decreasing due to avoiding distractions. Client takes initiative to help others during group session. Client was given permission to sit next to his friends while doing so. Client expresses positive and pleasant feelings toward peers and authority, at this time. Client engages in group discussion. Client shares his daily experiences at school in order to reflect and debrief during session. Client inquired about group activity. Client has implemented interventions and strategies taught by QMHP in his daily activities so that he shows different things he is learning in each. Client was applauded for his support and commitment in maintaining his skill sets needed to keep him motivated in school, at home, and in the community.
